In certain cases and within certain limits, a fee may be refunded by Nulton Diagnostic & Treatment Center. A refund may be made if the Agency is in error. A written request for the refund must be submitted to Nulton Diagnostic & Treatment Center, Attn: Billing Department, 214 College Park Plaza, Johnstown, Pa. 15904. Refunds are paid by Check only and may take up to 4-6 weeks to be issued.
